Day Hab Specialist

Seeking a Full Time (35 hr/wk) Day Hab Specialist split between our Pembroke & Franklin programs.

The bulk of this position will be working (in the Concord area) with a gentleman with challenging behaviors and some medical supports will be needed, while he pursues and participates in a variety of activities within the Concord community.

 

Nurse – New Adult Day Services Program for People with Dementia

Seeking a Part Time Nurse for this innovative new program.  Responsibilities include initial and ongoing nursing assessments; nutritional monitoring; supervision of medication administration and health-related issues; and medication administration training.

 Requirements: Current New Hampshire Registered Nurse license and minimum of one year experience working with older adults or persons with disabilities.

 

Nurse Trainer - Developmental Disabilities

Part Time Nurse Trainer for people with Developmental Disabilities. The Nurse Trainer is responsible for the supervision of medication administration and health-related issues; compliance with the requirements of He-M 1201 and other applicable regulations; medication administration trainings, including medication administration observations and re-certifications; development of nursing plans of care for residential group home clients and other clients as needed.

 Requirements include current New Hampshire Registered Nurse license and experience within developmental disability system is preferred.

 

Activities Coordinator for Adult Day Services Program for Individuals with Dementia

Seeking a Full Time Activities Coordinator (of Adult Day Services). We are looking for someone with previous experience developing and/or providing social, recreational and educational programming in a health care or social service setting. 

A bachelor’s degree is preferred; experience working with older adults and/or individuals with dementia required.

 

Home Provider

We are in search of a person/couple in the Concord/Franklin area to provide a safe residential placement for a man with developmental disabilities. The home provider(s) must be able to provide a structured environment.  The ability to apply behavioral intervention skills and previous experience working with developmentally disabled adults is required.

Residential Care Specialist

Full Time (32 hours/week; hours include weekend shifts) - Residential Care Specialist for our Franklin, NH program.

 

Personal Care Specialists

Full & Part Time Positions Available

Concord, NH

We are seeking Personal Care Specialists to assist with supporting the elderly and/or developmentally disabled in their home. Depending on the needs of the individual the position would require direct care and meal preparation along with other assistance identified on an individual basis.

 The desired candidates would be flexible with scheduling and daily assignments, have the ability to work independently,   the ability to individualize the supports needed for the individuals we serve and the ability to work with a variety of the individuals we serve on a daily basis.

 This position requires a minimum of a high school diploma/GED, previous human service experience preferred, reliable transportation, a valid NH driver’s license and vehicle insurance that includes 100/300 coverage.  A criminal and motor vehicle background check will also be required if an offer of employment is made.
