Seeking a Full Time (35 hr/wk) Day Hab Specialist split between our Pembroke & Franklin programs.
The bulk of this position will be working (in the Concord area) with a gentleman with challenging behaviors and some medical supports will be needed, while he pursues and participates in a variety of activities within the Concord community.

Seeking a Part Time Nurse for this innovative new program. Responsibilities include initial and ongoing nursing assessments; nutritional monitoring; supervision of medication administration and health-related issues; and medication administration training.

their home. Depending on the
needs of the individual the
position would require direct
care and meal preparation along
with other assistance identified
on an individual basis.
The desired candidates
would be flexible with
scheduling and daily
assignments, have the ability to
work independently, the
ability to individualize the
supports needed for the
individuals we serve and the
ability to work with a variety
of the individuals we serve on a
daily basis.
This
position requires a minimum of a
high school diploma/GED,
previous human service
experience preferred,
reliable transportation, a valid
NH driver’s license and vehicle
insurance that includes 100/300
coverage. A criminal and motor
vehicle background check will
also be required if an offer of
