
Our Franklin
Falls Farm residence is a beautiful old farm house located in
Franklin, New Hampshire. It offers both the serenity of the
country as well as the advantages of a community and town to its
residents.
All
the residents are survivors of acquired brain injury. The
program is designed for individuals who have completed
traditional rehabilitation services, yet continue to need
assistance for cognitive, vocational, social and independent
living skills.
An
individualized plan is implemented for each client. This plan
includes money management, menu planning, cooking skills,
recreational pursuits, and long term planning. If needed, the
plan will also focus on occupational, physical, and speech
therapies as well as behavioral services. The goal is to assist
each individual in reaching their maximum independence level and
also in returning to their community.

hour personalized care from a highly skilled staff who have
received specialized training to meet the clients needs.

Since
1982 the Enhanced Family Care program is a way of caring for and
about developmentally disabled people. The program goal is to
find an individual or family who is willing to share a home with
a developmentally disabled person. The Home Providers all
receive training from our staff so that they will be able to
give these disabled persons the training and personal attention
they need.
Mission Statement

meet the needs of those individuals who wish to live in the community within a "family setting" and receive support and
guidance in daily living.
Home care providers are carefully selected, trained and
supervised to assure quality services to residents.
The
Enhanced Family Care staff work with applicants to create a
compatible match and provide ongoing clinical and administrative

parts of Hillsboruogh counties serving clients ranging in age from mid-20's to
mid-80's

Located on Pleasant Street in Concord, New Hampshire this
residence is a state certified home currently providing a home
for three men, all of whom have complex medical needs such as
Huntington's disease, cerebral palsy etc...
The
program specializes in addressing the medical, behavioral,
nutritional, and personal safety needs of each resident. To
assure individual attention, each resident has an individualized
day program, which provides training in community living, self
care and socialization skills.
The
residence is fully accessible and provides 24 hour personalized
care from a staff who have received specialized training to meet
the clients needs. Transportation of clients is provided,
enabling the residents greater community involvement and access
to family and friends.

Hope Avenue is an assisted living facility
situated on a quiet residential street in Concord, New Hampshire and offers such
amenities as an enclosed yard, air conditioning, and complete handicapped
wheelchair accessibility for its residents.
This residence is a state certified and
licensed home currently providing a home for three men and three women, all of
whom are elderly and have complex medical needs. With transportation provided,
these individuals have easy access to community activities.
The home is fully accessible and provides 24
hour coverage that includes personalized care from a highly skilled staff who
have received specialized training to meet our clients needs. |
