CSCNH specializes in providing customized services to meet each person's medical, behavioral, and personal needs. Our goal is to assist individuals in reaching their maximum independence level and reintegrating them in the community.

Day programs may provide supportive assistance at a work site, educational classes and training in basic skills needed to return to the workplace, and self-care to assist in a transition to independent living, or simply opportunities to volunteer, pursue a hobby, or attend a community event with friends.

Franklin Falls Farm
Day Program

Located on 17 scenic acres
overlooking the Pemigiwasset River
in Franklin, New Hampshire

Most people with an acquired brain injury were employed prior to injury. Our goal is to assist them in as smooth and successful a transition as possible in returning to the workforce. Our staff recognizes the importance of finding the right job for each of our clients and provides the necessary training and support, placing a special emphasis on job readiness.

Employment can be one of the greatest sources for developing self-esteem. This is why it is so important to find the right job for each of our clients and provide them with the training and support necessary to succeed. Because each individual's interests and capabilities vary, we offer a variety of options ranging from consumer run businesses to individual job placement within the community. Our involvement with local businesses has also gained us their support and provided many job opportunities for our clients.


Located in Pembroke, New Hampshire this certified program serves individuals with challenging behavioral issues. It provides a variety of vocational opportunities, community based recreational activities as well as functional activities at the program site.

Our staff members work closely with the residential providers to monitor the challenging behavioral issues. The goal of this program and it's staff is to assist our clients in learning job skills in hopes of acquiring paid employment.
